The Asthma Action Plan: What Goes In It and How to Use It

Quick answer

An asthma action plan is a written guide made with your clinician. It lists your daily medicines, what to do when symptoms rise and when to call for emergency help. Zones, often green, yellow and red, show which step to take. Share it with family, school or work and review it yearly.

What the plan is for

Asthma changes from day to day. A plan means you do not have to decide what to do in a hurry, when you are short of breath. It sets out the steps in writing. Studies of asthma care show that people with a written plan do better at keeping control (CDC, About Asthma).

The three zones

Most plans use colours like a traffic light, though forms differ.

Zones
ZoneWhat it meansWhat to do
GreenDoing well, no symptomsTake daily controller medicine as directed
YellowSymptoms rising: cough, wheeze, tight chestUse quick-relief medicine and follow the written steps, call the clinic if no improvement
RedSevere: hard to talk, lips blue, rescue medicine not helpingUse rescue medicine and call 911

What goes in it

A good plan is specific, and it reads like a short set of instructions rather than general advice.

Your name and contacts, the names, doses and times of your medicines, your triggers, a peak flow range if you use a meter, symptoms for each zone and phone numbers for your clinic and emergency contacts.

How to build one

Ask your clinician at your next visit. Bring a list of your medicines and the times you use the rescue inhaler. Ask for a printed plan, and take a photo of it with your phone. Review it after any attack or medicine change. Ask the clinician to walk through each zone with you, and to explain in plain words what counts as no improvement, so you do not hesitate when it matters.

Use it as written

A plan only works if you follow it. Put a copy on the fridge, in your bag and in your phone. Show it to a family member.

Sharing the plan

Give a copy to your employer, a teacher, a coach, a babysitter and anyone who spends time with you or your child. They should know where the inhaler is and when to call for help.

What a plan does not include

Supplements are not on a standard plan. If you add one, tell your clinician, who can check for interactions. Do not replace any step in the plan with a capsule.

Peak flow and symptoms

Some people use a peak flow meter to track airflow. A fall in readings can warn of trouble before symptoms. Others use symptom diaries. Pick one method and use it every day for a while to learn your pattern (NHLBI, Asthma Symptoms).
